Step 2. Install WPForms Survey Addon

Next, you can go to the WPForms » Addons page. From there, you should look for Surveys and Polls Addon.

Click on the Install Addon button and then click on the Activate button.

Step 3. Create an NPS Survey Form in WordPress

Now you can set the score to 9 in the last field with the less than option. This field will only show when a customer selects a score below 9 on the scale.

Step 5. Display the NPS Survey Form in WordPress

To display the form, you need to visit Pages » Add New in your WordPress admin area.

The page editor where you can click on the plus icon and search for WPForms block. Click on it to add the WPForms block to your page.

It’ll show a drop-down menu where you can select the NPS survey form.

Now you can publish your page. Visit the website to see your NPS survey form in action.

Step 6. Monitor NPS Survey Form Results in WordPress

After getting a few responses, you can take a look at the survey results in WordPress.
