WPForms comes with a drag and drop form builder to create a contact form, survey forms, and any other type of form easily.

It also provides a Likert scale template that you can use with all the other features t0 create a questionnaire without writing any code.

Let’s get started with the step by step process to create a Likert scale questionnaire using WPForms.

Step 1: Install and Activate WPForms Plugin and Addons

The first thing you need to do is to install and activate the WPForms plugin. For more details, you should check out our guide on how to install a WordPress plugin.

Upon activation, you need to go to the WPForms » Settings page in the WordPress admin area to verify your license key.

To create a Likert scale survey questionnaire, you need the WPForms Pro plan or above. You can find the license key on your WPForms account.

After successful verification, you need to visit the WPForms » Addons page to install the Surveys and Polls addon.

Next, you also need to install and activate the Form Templates Pack addon.

Once both addons are activated, you’re ready to create a Likert scale questionnaire in your online survey form.

Step 2: Add a New WordPress Form With Survey Template

To add an online survey form, you need to visit the WPForms » Add New page in the WordPress admin area.

Start by adding a name to your new form. Let’s call it Survey Form, which you can change later as you like.

Next, you need to scroll down to the Advanced Templates. WPForms offers hundreds of ready-made form templates to set up any WordPress form in just a few clicks.

In the search box, enter ‘survey form’, and it’ll display the WPForms survey form template.

Click on this form template, and it’ll take a few moments to load the ready-made survey form with dummy questions.

Step 3: Add Likert Scale Questions to Your Survey Form

Now, you can customize the survey form to add a Likert scale. It shows the fields and field options on the left side and the preview of your survey form on the right side of the screen.

On the left side, you should look for the Fancy Fields section. From there, you need to drag and drop the Likert Scale field to the right side of your template.

To edit the Likert Scale, you should click on it, and it’ll show the settings on the left side of the screen.

These settings include the label or title of the questionnaire, rows where you can add your questions, and columns for the rating scale.

First, you should go ahead and enter your title for the questionnaire in the Label field. Then, under the Rows section, you need to add your question statements.

You can add as many statements as you need by clicking on the plus (+) icon.

We’ll leave the Columns section to the default rating scale choices. However, you can also extend it by clicking on the plus (+) icon. Make sure that the total count of answer choices remains an odd number.

Step 4: Change Likert Scale Template Settings

You can scroll down to the Advanced Options section to customize the Likert scale template.

It lets you enable survey reporting to track users’ input on the questions, change the Likert scale style (modern or classic), select the field size (small, medium, large), and manage custom layouts.

Once you’re done with the setup, click on the Save button at the top right corner of your screen to store these settings.

Step 5. Display Your Likert Scale Questionnaire in WordPress

Now that your survey form is ready, you should go ahead and display it on your WordPress website. Click on the Embed button from the top right corner.

It’ll open a popup where you need to select an existing page or create a new page to publish the survey form.

Let’s create a new page.

On the next popup screen, you need to add a name for your new page. And click on the Let’s Go! button.

It’ll open the page editor with the survey form and Likert scale that you added. Once you’re ready, click on the Publish button.

Now you can visit your website to see the Likert scale questionnaire in action.
